Info For Parents
If you are enrolling your child/children in a FCCC program please download, print, and complete all of the forms. Completed forms can be submitted to the centre supervisor at the site you are enrolling in.
Immunization Records
Along with these registration forms, you will need to provide a copy of your child's up-to-date immunization record unless your child is enrolled in school. If your child is not up-to-date with their immunizations you can contact the KFL&A Public Health for immunization clinic times or see your doctor.
